DOT Physicals

Physical includes blood pressure, vision, hearing, and a basic urinalysis. Drivers receive a laminated certificate and exam (long form) $80.00
Re-exam (less than 1 year expiration) $55
Drivers that need additional glucose test: additional $5.00

Electronic exams can be used as well. We can have forms filled with tablets and sent PDF or printed upon completion. Exams are immediately reported to the FMCSA via e-screen or FormFox services.

DOT/Non-DOT Drug Screen

Urine testing for marijuana, cocaine, (meth)amphetamines, PCP, and Opium. 2018 tests include analytes oxycodone, oxymorphone, hydrocodone, and hydromorphone
$15.00 Collection fee (with established lab account/MRO) Drivers or employees have a pre-printed CCF form and only need collection and shipping.
$60.00 Collection/Lab/MRO fee using our Interstate Chiropractic account for those without an established lab account and we provide results e-mail, fax, or mail. Results typically 1-2 days.

Drug screens need to be completed no later than 6:00PM M-Th and 4:00PM Friday

Alcohol Screen

Breath Alcohol Screen using latest Alcovisor Jupiter BAT unit. DOT/FMCSA approved confirmatory test. $25.00 in-house with immediate results

Walk-Ins are welcome for any services, however, appointments are encouraged and receive priority. Services are available during any of our business hours. M-Th 9:6:00, Fri 9:00-4:00
